Annual Report Form Partnership Activities

As in the past, the purpose of the Annual Report Form is to gather information about professional development programs conducted by Partnership Teams during the most recently completed academic year. A comprehensive report, prepared by the Kennedy Center from Teams’ data, is shared with the US Department of Education which funds the Partners in Education program, as well as with the entire Partners in Education network. This data helps to demonstrate our collective impact, and Partnership Teams state that they learn about new workshops and workshop leaders through the report of Team activities.

To complete the form, answer the following questions and click “submit” at the bottom of the page. Be sure to print out a copy of your completed form for your records before submitting. Thank you.

*Definitions:

Sampler Courses: A series of workshops, each on a different topic or different art form.

In-depth Courses: Multiple, sequential, interrelated sessions examining one topic in-depth.

Demonstration Teaching: Teaching artist demonstrates an arts-integrated strategy in the classroom while the teacher observes.

Coaching: Teaching artists work one-on-one with a teacher to observe his/her teaching of an arts-integrated strategy in the classroom and provide feedback.

Study Groups: Teachers meet in a small group to discuss topics related to arts integration.
